Terms and conditions for the Use of SMS

These terms and conditions regulate the use of text messaging services (SMS) provided by HPM Foundation, Inc. as a method of communication with its patients.

By accepting to receive SMS from HPM Foundation, Inc you acknowledge that you read and agree to the terms and conditions.

1. Purpose of the SMS
The SMS will be sent for the following:
- Appointment reminders
- Notification of Lab or Procedure results.
- Information about your treatment plan or care plan.
- Administrative communications, such as changes in service hours or working hours.

2. Consent
When you provide your phone number you:
- Authorize HPM Foundation, Inc. to send SMS messages.
- You confirm that the provided phone number is under your control and can receive SMS messages.

3. SMS Charges
HPM Foundation, Inc. doesn't charge for SMS, however your phone service provider might apply charges for SMS. This depends on your phone plan or contract.

4. Confidentiality:
We do all reasonable safeguards to ensure that the information sent by SMS is safe and secured. However, due to the nature of the means of delivery, we can't 100% guarantee the privacy of SMS messages. For this reason, sensitive information will be limited in the SMS sent.

5. User Responsibility
You're responsable for:
- Making sure the phone number is updated.
- Notifying HPM Foundation, Inc. about any changes in the phone number or if you decide to opt out of the service.

6. Responsibility exclusion
HPM Foundation, Inc. is not responsible for:
- Delays or failures in the delivery of SMS messages due to technical problems, carrier problems or any other circumstance out of our control.
- Unauthorized access to the SMS once they have been delivered to your device.

7. Cancelation of SMS Service
You can opt out of receiving SMS at any time by answering "BAJA" to the that sent you the messages or by directly contacting HPM Foundation, Inc

HPM Foundation, Inc reserves the rights to modify these terms and conditions at any time.
